D-ribose is a key player in the synthesis and regeneration of ATP (adenosine triphosphate). D-ribose appears to have a significant clinical role in cardiac health, specifically related to myocardial contraction during acute depletion and increased demand for ATP. Athletes report improved speed and recovery following intense exercise. D-ribose may also help with chronic fatigue, muscle tenderness, joint sensitivity, and lethargy. D-ribose is a 5-carbon sugar involved in the maintenance of ADP and AMP, necessary for the salvage pathways of ATP resynthesis; it is also the rate-limiting step for the de-novo resynthesis pathway of ATP. Endogenously, d-ribose is produced by the hexose monophosphate shunt. Still, in times of increased need for ATP, exogenous d-ribose has been shown to replenish critical ATP levels promptly. In animal studies, ATP levels were restored following an ischemic event in less than two days, compared to the usual ten days when given oral d-ribose. Endurance or high-intensity athletes can support their ATP levels by taking d-ribose during or after exercise.
Five grams is a standard dose. The typical range of intake is 5-20 grams per day. It is not recommended to take more than 5 grams in any 2 hours; more than 5 grams at once can cause a transient decrease in blood sugar. Research in individuals with chronic fatigue and muscle tenderness indicates improvements with 5 grams of d-ribose 3 times a day.
